Name Change and Data Corrections

All regular American Citizen Services (ACS) are by appointment only.  Please click on Online Appointments to make your ACS appointment online.  We suggest you read the following requirements before you make an appointment for passport services.

Name Change

If you have legally changed your name after your passport has been issued you will need to follow one of the procedures below to have your passport re-issued in the correct name or replaced:

If the name change occurred one year or less from the date of issue of your current passport please submit:

  • Your current valid passport.
  • Completed application for data correction, name change, or extension of limited validity passports (DS-5504). All passport forms are available here.
  • One photograph for the application. Click here for more information.
  • Documentary evidence of the name change (can include court order, marriage or divorce certificate, or other satisfactory evidence of name change)
  • You should bring legible photocopies of all the supporting documents with you.
  • No fee is assessed for this service.

If the name change occurred more than one year from the date of issue of your current passport please submit:

  • Your current valid passport.
  • Completed application for data correction, name change, or extension of limited validity passports (DS-5504).  All passport forms are available here.
  • Completed online application for a renewal of a U.S. passport (DS-82). You must complete and submit the application online then print a copy to bring to your appointment.  All passport forms are available here.
  • One photograph for the application. Click here for more information.
  • You should bring legible photocopies of all the supporting documents with you.
  • Documentary evidence of the name change (can include court order, marriage or divorce certificate, or other satisfactory evidence of name change).
  • Passport Fee .  Fees must be paid in cash, and are payable either in United States Dollars or in equivalent Bangladeshi Taka.
    Post does not provide any File Search Service, Expedite Service and Overnight Delivery.  

Data Correction

If you, at any point in time, determine that there is a printing error on your passport you will need to apply for a new passport by submitting the following:

  • Your current valid passport.
  • Completed application for data correction, name change, or extension of limited validity passports (DS-5504).  All passport forms are available here.
  • One photograph for the application. Click here for more information.
  • Documentation supporting the data mismatch (for example, certified birth certificate, original marriage or divorce certificate, etc.)
  • You should bring legible photocopies of all the supporting documents with you.
  • No fee is assessed for this service.
